Rachel D. Velazquez

Rachel grew up in Rigby, Idaho.  She watched several family members return from surgeries and injuries and knew she wanted to pursue a career in physical therapy to help others regain and maintain their independence and their return to their hobbies, work, and passions. Rachel moved to Logan, Utah where she completed her undergraduate degree in Human Movement Science at Utah State University. Throughout her coursework, Rachel’s love and appreciation for the human body grew and solidified her goal of pursuing her physical therapy degree.

In her spare time, Rachel enjoys going to the gym with her husband, horseback riding, hunting, and spending time with family. In April 2025, Rachel and her husband welcomed a beautiful baby girl into their lives and have enjoyed this new chapter of their lives.

Rachel loves getting to know her patients and hearing about their goals, passions, and life experiences. Educating and empowering patients to be active participants in their healthcare journey to improve their quality of life is one of Rachel’s many passions. Rachel has experience in outpatient orthopedics and aquatic therapy, inpatient acute care, pediatrics, and pelvic floor therapy. Rachel has special clinical interest in treating women’s health. Rachel has had training in Pregnancy, Postpartum, Pelvic Floor and Diastasis Recti with Sarah Duvall. She has also had training in Pelvic Floor with Herman & Wallace, Pelvic Rehabilitation Institute, Graston Technique, and Dry Needling.

She is excited to continue learning and developing her skills to stay up to date on the most recent research and evidence to provide the highest quality treatment.
